Safety Concerns

The mail-in program will still need to follow all of the defined APS/DOE safety policies. The requesting institution will provide a Hazard Assessment Form by either e-mail or fax to SER-CAT at 630-252-0652. The staff will prepare the ESAF for the time slot just as any user. The staff will post the ESAF with a sample naming protocol of MIC-##. The staff will need to generate a sample form that has the name of the protein cross indexed to the ESAF. The sample form will be kept in a locked location within the SER-CAT office module just like any other safety documentation.

If you are not willing to divulge the name of your protein, then that sample cannot participate in the mail-in program. We are bending the rules slightly by not identifying the sample 2 weeks in advance (by new DOE regulation). Therefore, we must identify the protein.
